Membership Form
This is the form you'll need to join UPHA. If you wish to accumulate points for year-end awards, you must be an active member before you show & must make 3/4 of all shows.
Horse Registration Form
This is the form that you need to fill out to register your horse with UPHA. This is necessary if you wish to accumulate points for year-end awards.
